If your birth certificate has been proven unavailable or insufficient, then there are two things that the government agency may request for: a certificate of non-availability and a birth affidavit.
The birth affidavit would usually come from individuals who were alive at the time of your birth, or were aware of it during that specified moment. While this is usually signed by the applicant’s parents, this could also be obtained from an aunt, uncle, sister, brother, or any older blood relative who was present at the time of the applicant’s birth.
Generally, an affidavit of birth must contain the following information:
- An applicant’s personal background – The document must state the applicant’s full name, gender, and date of birth. Be wary of spelling mistakes or missing details that might raise questions. You could also record things such as the subject’s place of birth and possible changes to make it easier for the agency to trace the applicant.
- The full names of the applicant’s parents – The full names of the applicant’s parents must also be stated clearly in the affidavit. Even if the document was executed by someone other than one’s parents, the same rule still applies. - The relationship between the writer and the subject – The affidavit must indicate how the writer knows of the applicant’s birth. Naturally, a mother would state that she knows about the birth because she’s the one responsible for bringing you to this world. But if the father or another family member happens to sign the affidavit, then they could indicate that they witnessed the birth while at the hospital. A medical personnel may also be authorized to sign the affidavit, but some officials highly discourage this as their claims can sometimes be questionable.
- The complete names of everyone stated in the affidavit – Always remember to state the complete names of every single person that appears in the affidavit. This includes the applicant, the parents of the applicant, and the person responsible for signing the affidavit. Avoid using a person’s initials even if this is your nation’s naming custom, otherwise, it would be difficult to trace the names listed in the document.
