Critical Incident Report

On May 15, 2024, at approximately 3:45 PM, a critical incident occurred at the manufacturing facility of Apex Industries, located at 100 Industrial Way, Techville, TV. The incident involved a chemical spill in the plant’s processing area, where a ruptured storage tank released approximately 200 gallons of hexane, a highly flammable liquid.

The facility’s emergency response team was immediately activated upon detection of the spill. The area was evacuated, and local fire and hazardous material (HAZMAT) teams were alerted and dispatched to the scene. The spill triggered the facility’s fire suppression systems, preventing ignition of the spilled chemical.
