Critical Incident Report

On May 15, 2024, at approximately 3:45 PM, a critical incident occurred at the manufacturing facility of Apex Industries, located at 100 Industrial Way, Techville, TV. The incident involved a chemical spill in the plant’s processing area, where a ruptured storage tank released approximately 200 gallons of hexane, a highly flammable liquid.

The facility’s emergency response team was immediately activated upon detection of the spill. The area was evacuated, and local fire and hazardous material (HAZMAT) teams were alerted and dispatched to the scene. The spill triggered the facility’s fire suppression systems, preventing ignition of the spilled chemical.

Preliminary investigations suggest that the rupture was due to a structural failure of the storage tank, potentially caused by corrosion. Samples of the tank material have been collected for further analysis to confirm the cause of the failure.

Two employees, John Doe and Jane Smith, were in the vicinity of the spill at the time of the incident. Both were exposed to hexane vapors before the evacuation. They were taken to the local hospital for evaluation and treatment of potential hexane inhalation. Both employees were kept under observation for 24 hours as a precautionary measure and were released with no significant health issues reported.
