The purpose of a survey is to simply gather data in hopes that the institution will be able to learn newer things and conditions not only with regards to their clientele or to their research participants. There are certain surveys that are intended for academic purposes only (e.g. thesis related). While there are others that conduct simple surveys in order to understand a certain situation and how it can be controlled should the need arise.

For instance, it is important for parents and other concerned parties (e.g. government officials handling the use of technology, medical centers, doctors, psychologists, etc…) to try and understand as to whether that person is using the Internet wisely or is he rather addicted to it.

People who conduct the survey understand the need for confidentiality and discretion which is why they would normally not include your basic information other than your sex and your age in their survey since those two aspects are essentially the heart and soul of your data. Without them, you would just have random answers without really knowing anything.
