I hope this letter finds you in good health and spirits. My name is Winford Dickerson, and I am a student in the 11th grade, enrolled in the Science stream under the mentorship of Mrs. Lydia Hart. I am writing to request permission to sit for the Physics midterm exam, which was held on February 8, 2024.

Unfortunately, due to a sudden illness that resulted in my hospitalization, I was unable to attend the scheduled exam. I have attached a medical certificate from Dr. Allen Greene, confirming my condition and the necessity of my hospital stay during the exam period.
